Rates and insurance

Psychotherapy Fees:  

Individual Psychotherapy: $220 per 55 minute session

Initial assessment: $250

Psychological Assessment:   

Fees based on diagnostic issues and testing focus. 

Assessment for ADHD: $2300, not billable to insurance.  Superbill can be provided. 

Family Reconciliation Costs: 

60-minute consultation: $200

90-minute consultation: $300

These meetings are not billable to insurance.

Insurance:  

Services may be covered in full or in part by your health insurance.  I will submit claims to your insurance company as a courtesy. Please check your coverage prior to making an appointment.  I am currently accepting the following insurance:

I am able to bill other insurance companies as an out-of-network provider for therapy 

Important questions to ask your insurance include:  

1 . What are my outpatient mental health benefits?  

2 . Do I have a deductible and has it been met?  

3 . What is my co-pay or co-insurance?  

5.  Is Susannah Castle an in-network provider with my insurance plan? 

6.  Do I have out-of-network benefits, and what are they?  

Please bring your insurance card with you to your first appointment. 

Payment: Co-pays are due at the time of service. Co-insurance will be billed after insurance payments are made. I accept cash, credit or checks. Clients are responsible for all fees not covered by insurance.  I use a Square app to maintain confidential credit card information on file.

Cancellation Policy:  I have a 24-hour cancellation policy.  Cancellations received with less than 24 hours notice will be charged a $125 fee.
